A critical aspect in transplantation is the choice between cadaveric and living donor transplantation. Living donor transplantation may offer several advantages over cadaveric transplantation, but it has been advocated only in selected indications. To describe the indications and results of living donor liver transplantation (LDLT) in Spain. We reviewed the literature on LDLT in Spain and also analysed the Spanish data published on LDLT in the last 10 years. A group of specialised hepatobiliary and transplant surgeons conducted three consensus meetings on the subject of LDLT in Spain in recent years (2001, 2003 and 2006). To analyse the Spanish evidence, we took into account the variables of donor, recipient, follow-up and outcome. LDLT in Spain has been applied for patients with end-stage liver disease and in certain benign diseases. Hospital survival was good in all studies published for LDLT in Spain (91-100%) and also for cadaveric transplantation (100%). Clinical results appeared to be good and the survival rates similar to cadaveric transplantation. Living donor liver transplantation in Spain is mostly limited to the paediatric population in specialised centres with low morbidity and excellent results, although there are recent results to confirm the feasibility and safety of LDLT in adults with end-stage liver disease.Q:
