In this study, we have attempted to clarify the optimal length of the cystic duct for common bile duct exploration to reduce the incidence of postoperative pancreatitis. Two hundred thirty-six patients underwent cystic duct dissection for the investigation of suspected cystic duct stones. There were 147 men and 89 women, and the mean age was 60.8 years. The subjects were divided into three groups according to the length of the cystic duct: group A (n = 87), the cystic duct was dissected to the duodenal papilla; group B (n = 81), the cystic duct was dissected to within 10 mm of the duodenal papilla; and group C (n = 58), the cystic duct was dissected to within 5 mm of the duodenal papilla. Operative findings were compared among the three groups. Postoperative complications were identified in 15 patients (5.9%), including two cases of pancreatitis (0.8%). There was no significant difference in the incidence of postoperative pancreatitis among the three groups (p > 0.05). Postoperative complications were more frequently observed in patients with a cystic duct longer than 10 mm, and the incidence of pancreatitis in these patients was significantly higher than in those with a cystic duct shorter than 10 mm (p < 0.05). We believe that a cystic duct longer than 10 mm is a risk factor for the development of postoperative pancreatitis and that the cystic duct should be dissected to within 5 mm of the duoden
