The aim of this study is to compare the oncological outcomes of patients treated by Transoral Robotic Surgery (TORS) with conventional laryngeal procedures and to investigate the safety of TORS in patients with laryngeal malignancy. Studies comparing TORS and non-TORS treatment in patients with laryngeal malignancy were identified via an electronic search using PubMed, Cochrane Library, EMBASE, Scopus, and Web of Science databases. The primary endpoints were the oncological outcomes and the perioperative and postoperative complications. Six studies involving a total of 1107 patients were identified. The combined analysis demonstrated that TORS was associated with a significantly decreased recurrence rate at a mean follow-up period of more than 2 years, especially for early cancer and subsite recurrence in patients with laryngeal malignancy. The oncological outcomes of TORS were not inferior to those of non-TORS for both early and late-stage cancer. TORS was associated with significantly less intraoperative blood loss.
